Patient Visit Policies

24 Hour Cancellation Policy:

Cancellations made within 24 hours will be charged the full visit amount with no exceptions.

We require notice 24 hours prior to your appointment if you need to cancel or change your time. To accommodate all our patients, we do maintain a cancellation list. If you do need to cancel or reschedule your visit, please give us adequate notice as someone else can benefit from the time.

If you fail to show up for a scheduled appointment without notice, you will be charged the full visit amount.

We require credit card number or prepayment to book/hold your initial appointment. If you need to change your appointment, and more than 24 hours is given prior to your scheduled appointment, your prepayment will be applied to your rescheduled visit.

If you are booking a visit within 24 hours of the appointment time, there is no cancellation and you will be charged the full amount if the appointment is missed.

Late Arrival Policy:

As a courtesy to our other patients and staff, appointments will be automatically cancelled 20 minutes after scheduled time and charged according to our cancellation policy. We regret that late arrivals will not receive extension of scheduled appointments. If late arrival is inevitable, your visit will be shortened in order to keep on schedule.

Only in special cases, and when our schedule will allow, we may be able to accommodate a partial or full appointment. This will be at the doctor’s discretion and only with proper, advanced notification of your late arrival.

Accounts and Collections:

Payment is expected at the time of treatment. On the event a payment is missed, the office will attempt to call and email. If we are not successful in contacting the account holder, we will send out an invoice, payment due upon receipt.

Accounts with a delinquent balance of over 90 days will be sent to an outside collection agency with reporting authority to Equifax and TransUnion. They will also be reported to the Credit Bureau.

We accept cheques for the convenience of our patients, they can be made payable to Dr. Marie-Claude Bozek. For cheques that are return NSF there will be a $25.00 charge.

Refund Policy:

We do not offer refunds on Chiropractic services after the fact. Refunds will not be provided for deposits or monies paid on cancellations received less than 24 hours before a scheduled appointment.

For products ordered or supplied by Dr. Bozek’s office, we respect and adhere to the manufacturer’s or supplier’s return and refund policies.

These policies were adopted to ensure that Dr. Bozek’s time, the office and its efforts be respected, as well as your scheduled appointment be a stress-free experience.
